What Is Support Coordination?
Support Coordination is a funded service under the NDIS that helps participants:
Understand and implement their NDIS plans
Connect with NDIS-registered and community providers
Build capacity to manage services more independently
Navigate service agreements, budgets, and progress reviews
Prepare for plan reassessments or changes
Our goal is to make your NDIS journey less stressful, more effective, and fully aligned with your life goals.
What Our Support Coordinators Do
Our coordinators act as your personal NDIS guide and advocate. They work closely with you and your support network to ensure you feel informed, in control, and supported.
We help with:
✔️ Understanding each part of your NDIS plan
✔️ Sourcing and coordinating quality services
✔️ Setting up service agreements and bookings
✔️ Monitoring the effectiveness of your supports
✔️ Resolving issues with providers, if they arise
✔️ Linking you with informal, community, and mainstream supports
✔️ Assisting with plan reviews and preparing reports

Who Can Access This Service?
You may be eligible for Support Coordination if your NDIS plan includes funding for Support Coordination or Coordination of Supports.
